The Collision Investigation Field notepad was developed to assist field personnel in taking adequate notes to generate a comprehensive report back at the office while maintaining the confidence that all required information was obtained. Starting with the front cover and continuing through the notepad to the back cover, every detail has been thought of and scrutinized for its pertinence in the field. The cover is yellow to distinguish the notepad as a Collision notepad The cover is laminated to resist the elements The lower corners are rounded to ease inserting into pocket The front of the information pages include: Notification information Location information Involved Party information Insurance information Vehicle information Statement (minimal [driver, vehicle, highway, direction, lane, speed, proximity]) The back of the information pages include: Injured Parties or Passenger information or Witness information Statement (minimal) Many have asked about other missing elements in this notepad. We have always reminded them that these notepads are intended for field notes. This notepad is typically used in conjunction with the Driving Under the Influence field notepad.
