Q: Is there a barrel adjuster only on the 3 speed shifter and not on the 7 speed shifter?
A: Yes, the barrel adjuster for the rear derailleur is at the derailleur, so that shifter doesn't need it, the shifter with the adjuster is for the front derailleur.

Q: This is for both the left and right?
A: Yes, 3 position shifter goes on left side of bar for triple crankset, 7 position shifter goes on right side of bar for 7-speed rear cog.

Q: Does the shifter come with the brake cables or just the gear?
A: Break cables are not included, but the shifter cables (which change the gears) are included, just like picture shows.
