This control has a black housing with six buttons; the top two show the back of the chair moving up & down, the next two show the footrest going up & down, and the last two are for the whole chair going up & down. It has a straight quick disconnect plug with five (5) pins. If the picture looks like what you have, then it will be the correct one.
